******省分行運(yùn)營中心***上半年食堂食材及用品配送項目（招標(biāo)人編號***標(biāo)代理編號***，******省分行批準(zhǔn)實施，項目資金來自企業(yè)自籌，資金已落實。******省分行，***（以下簡稱“招標(biāo)代理機(jī)構(gòu)”）對該項目進(jìn)行公開招標(biāo)。 2. 項目概況與招標(biāo)范圍 2.1 招標(biāo)內(nèi)容：本項目擬通過公開招標(biāo)的方式選擇1家供應(yīng)商，提供***省分行運(yùn)營中心***上半年食堂食材及用品（含食材、調(diào)味品、糧油等）配送服務(wù)。具體內(nèi)容及要求詳見招標(biāo)文件***：項目需求書。 2.2標(biāo)包劃分情況：本項目不劃分標(biāo)包。 2.3 項目預(yù)算：20***元。 2.4 ******省分行出資。 2.5 合同有效期：自******月***日起至******月***日，如合同簽訂之日晚于******月***日，則合同有效期為自合同簽訂之日起至******月***日。 3. 投標(biāo)人資格要求 3.1投標(biāo)人須為具有獨立法人資格的境內(nèi)企業(yè)或其他組織（如是分支機(jī)構(gòu)，則須提供具備獨立法人資格的上級機(jī)構(gòu)***），對于已按“三證合一”登記制度更換新版營業(yè)執(zhí)照的（營業(yè)執(zhí)照、組織機(jī)構(gòu)***、稅務(wù)登記證三證合一），須提供由工商部門頒發(fā)的加載統(tǒng)一社會信用代碼的營業(yè)執(zhí)照。 3.2具有良好的商業(yè)信譽(yù)。須同時提供下列截圖：（1）國家企業(yè)信用信息公示系統(tǒng)（網(wǎng)址：http***cn/index.html）中投標(biāo)人“列入嚴(yán)重違法失信企業(yè)名單（黑名單）信息”和“列入經(jīng)營異常名錄信息”的查詢結(jié)果截圖。（2）“信用中國”（網(wǎng)址：http***Page=4）“失信被執(zhí)行人”、“政府采購嚴(yán)重違法失信行為記錄名單”、“重大稅收違法失信主體”的查詢結(jié)果截圖；注：如投標(biāo)人因單位***，無法提供上述截圖的，則須提供相應(yīng)情況說明。 3.3 具有健全的財務(wù)會計制度，需提供***年度財務(wù)審計報告或***年度財務(wù)報表（包含資產(chǎn)負(fù)債表、利潤表及現(xiàn)金流量表）。注：因受成立年限影響，無法提供上述證明材料的，提供最近三個月任意一個月的財務(wù)報表。 3.4投標(biāo)人必須具備***至今的同類項目合作經(jīng)驗。（以合同簽訂時間為準(zhǔn)，要求提供合同關(guān)鍵頁復(fù)印件）。 3.5 資質(zhì)要求：投標(biāo)人必須具備有效的《食品生產(chǎn)許可證》或《食品經(jīng)營許可證》。需提供相關(guān)證書復(fù)印件。 3.6項目關(guān)聯(lián)性：因“***省分行運(yùn)營中心***上半年食堂后勤服務(wù)項目（簡稱：后勤服務(wù)項目）”和“***省分行運(yùn)營中心***上半年食堂食材及用品配送項目（簡稱：食堂食材及用品配送項目）”關(guān)聯(lián)性較強(qiáng)，兩個項目不兼投不兼中（即每個投標(biāo)人只能投標(biāo)、中標(biāo)其中一個項目，招標(biāo)人優(yōu)先對“后勤服務(wù)項目”進(jìn)行評標(biāo)，如投標(biāo)人同時參加“后勤服務(wù)項目”和“食材及用品配送項目”的投標(biāo)，則只接受“后勤服務(wù)項目”的投標(biāo)，“食堂食材及用品配送項目”的投標(biāo)將視為無效投標(biāo)）。投標(biāo)人須提供承諾函。 3.7 投標(biāo)人必須開具增值稅普通發(fā)票。注：投標(biāo)人需提供稅局相關(guān)證明文件或已開具的增值稅普通發(fā)票樣本復(fù)印件。 3.8本次招標(biāo)不接受聯(lián)合體投標(biāo)。 3.9利害關(guān)系： 3.9.1單位***、管理關(guān)系的不同單位，不得參加同一標(biāo)段投標(biāo)或者未劃分標(biāo)段的同一招標(biāo)項目投標(biāo)。須提供控股及管理關(guān)系情況申報表（格式見第六章）。 3.9.2投標(biāo)人與郵政無投資關(guān)系且存在以下情況的，不得參加本次招標(biāo)/采購活動：郵政領(lǐng)導(dǎo)人員及其親屬和其他特定關(guān)系人、郵政員工持股（***），以個人身份（組織委派的除外）擔(dān)任法人、董事長、總經(jīng)理、監(jiān)事的企業(yè)，以及郵政所屬工會或員工集體出資成立的企業(yè)。詳見投標(biāo)人不得存在情況承諾書。 3.10投標(biāo)人或其投標(biāo)產(chǎn)品/服務(wù)不得存在下述情況： 3.10.1被列入招標(biāo)人（***、******省分行）供應(yīng)商管理黑名單，且在禁入期內(nèi)被暫停或取消投標(biāo)資格的。 4. 招標(biāo)文件*** 4.1注冊登記凡有意參加投標(biāo)者，須在中國郵政儲蓄銀行-郵銀易采平臺（以下簡稱郵銀易采平臺，http***）申請注冊（具體方式可詳見郵銀易采-幫助中心-供應(yīng)商操作手冊），并在注冊成功后登錄賬號。 4.2購買文件 4.2.1招標(biāo)文件***：******月***日至******月***日，每日上午***時***分至***時***分，下午***時***分至***時***分（北京時間，下同） 4.2.2招標(biāo)文件***（￥500）整，售后不退。 4.2.3 招標(biāo)文件***（有以下兩種方式，可選擇其一進(jìn)行操作）： 4.2.3.1系統(tǒng)支付文件費(fèi)用：***電子招投標(biāo)平臺http***scm.cn完成招標(biāo)文件***，***統(tǒng)一社會信用代碼，登錄后請及時完善信息并更改密碼。網(wǎng)站主頁可下載《中捷招標(biāo)系統(tǒng)投標(biāo)人網(wǎng)上售標(biāo)操作手冊》參考流程。 4.3獲取文件潛在投標(biāo)人在郵銀易采平臺選擇采購業(yè)務(wù)-我的項目-，選定本項目，立即參與，進(jìn)行購標(biāo)（上傳文件費(fèi)用付款憑證），經(jīng)審核通過后，潛在投標(biāo)人即可在郵銀易采平臺（http***）平臺獲取電子版招標(biāo)文件，可參與后續(xù)投標(biāo)。 5. 投標(biāo)文件的遞交本項目采用郵銀易采平臺電子投標(biāo)和線下紙質(zhì)投標(biāo)并行，投標(biāo)人應(yīng)在郵銀易采平臺遞交電子投標(biāo)文件及線下遞交紙質(zhì)投標(biāo)文件，否則視為無效投標(biāo)。投標(biāo)文件（電子投標(biāo)文件和紙質(zhì)投標(biāo)文件）遞交的截止時間***（投標(biāo)截止時間***，下同）為******月***日***時***分（北京時間）。 5.1紙質(zhì)投標(biāo)文件的遞交：遞交地點：***市體***路一號峰源大***樓會議室；逾期送達(dá)的、未送達(dá)指定地點***、不按照招標(biāo)文件***，招標(biāo)人將予以拒收。 5.2郵銀易采平臺電子投標(biāo)文件的遞交：電子投標(biāo)文件須通過郵銀易采平臺遞交（注：成功遞交電子投標(biāo)文件的平臺狀態(tài)顯示為“遞交成功”）。逾期提交的電子投標(biāo)文件，系統(tǒng)將不予接收。郵銀易采平臺電子投標(biāo)文件須使用CA證書進(jìn)行加密后遞交。CA證書具體辦理流程參見中國郵政儲蓄銀行-郵銀易采上方“CA辦理”專區(qū)。本項目投標(biāo)文件的制作（包括簽章、加密等）、網(wǎng)上遞交、開標(biāo)（解密、確認(rèn)報價***件***、評標(biāo)澄清問題答復(fù)等環(huán)節(jié)均需要使用CA證書。請有意向投標(biāo)的供應(yīng)商在注冊時同步申請辦理，以免影響正常投標(biāo)。由于未及時辦理CA證書影響投標(biāo)的，后果由投標(biāo)人自行承擔(dān)。 5.3投標(biāo)保證金：￥24,0***元（人民幣***元整）。 6.開標(biāo) 6.1電子開標(biāo) 本項目將于投標(biāo)文件遞交截止的同一時間，在***市體***路一號峰源大***樓會議室，使用郵銀易采平臺開標(biāo)，邀請投標(biāo)人參會代表攜帶電腦、WIFI熱點、CA證書以及生成的秘鑰文件等準(zhǔn)時參加。 6.1當(dāng)電子投標(biāo)文件開標(biāo)解密正常時，開標(biāo)、評標(biāo)以電子投標(biāo)文件為依據(jù)。 6.2電子投標(biāo)文件解密異常處理 6.2.1電子投標(biāo)文件解密異常時，招標(biāo)人/招標(biāo)代理機(jī)構(gòu)***，若為系統(tǒng)故障原因，則推遲該項目的投標(biāo)截止時間***（具體時間另行通知）直至該投標(biāo)人完成電子投標(biāo)文件解密。因投標(biāo)人原因（如解密密碼輸入錯誤等）造成投標(biāo)文件未解密的，則視為撤銷其投標(biāo)文件。 6.2.2若因系統(tǒng)故障無法及時解決，則啟用紙質(zhì)招標(biāo)投標(biāo)程序。 7. 資格審查方式本項目采用資格后審辦法對投標(biāo)人進(jìn)行資格審查，由評標(biāo)委員會在開標(biāo)后按照招標(biāo)文件***。 8. 公告發(fā)布本公告或公告的修改、補(bǔ)充同時在中國郵政儲蓄銀行郵銀易采平臺（http***）、采購與招標(biāo)網(wǎng)（http***ding.com.cn）、中國郵政官方網(wǎng)站（http***t.com.cn）、***電子招投標(biāo)平臺（http***scm.cn）、中國金融集中采購網(wǎng)（http***m）以及中國招標(biāo)投標(biāo)公共服務(wù)平臺（http***rvice.com）上發(fā)布。本公告在各媒體發(fā)布的文本如有不同之處，以在中國郵政官方網(wǎng)站發(fā)布的文本為準(zhǔn)。
